Required Participant Forms for Camp/On-Campus Activities

Before your student can participate in their chosen activity, there are a few forms that must be filled out. Please take the time to complete and submit the necessary forms below by the first day of your student's activity/camp.

Activity Release and Indemnity Agreement

This release is required by 蜜桃视频 University. Students will not be able to participate unless the waiver is signed and submitted before the first day of their activity/camp.

COMPLETE MANDATORY RELEASE

Authorization to Dispense Medication

If your student will need to take medication while they are attending their activity at LU, you will need to contact the camp director to receive the appropriate form(s) to bring to the first day of your student's activity/camp.
